Pompey lifted the Checkatrade Trophy following a penalty shoot-out success over Sunderland.
They were not at their best before the break and deservedly trailed to a deflected Aiden McGeady free-kick.
But Nathan Thompson levelled to take the tie to extra-time and a glorious lob from Jamal Lowe looked to have won it for the Blues.
McGeady scrambled in an equaliser at the death, however, to ensure that spot-kicks would be needed and Kenny Jackett's men held their nerve.
Gareth Evans, Brett Pitman, Lee Brown and Jamal Lowe all scored, while Craig MacGillivray saved from Lee Cattermole before Oli Hawkins struck the winner.
